CONTINUING EDUCATION ACTIVITIES Last updated---Wednesday March 03, 2010 Bring your ASHA membership number with you to CE activities if you want ASHA CEUs. WHY? Beginning January 1, 2010, ASHA will no longer accept social security numbers on the ASHA Participant Form. The ASHA (or NSSLHA) account/ID number will be used. The ASHA account/ID number is most easily found on your ASHA membership card. The account/ID number can also be obtained by contacting the ActionCenter, 800-498-2071 or by e-mail: actioncenter@asha.org. Tips for keeping up with your ASHA account/ID number: 1. Cut the address label off an ASHA Leader and store it in your wallet or purse. It has your ASHA number on it. 2. Store your ASHA number in your cell phone under contacts: ASHA number. 3. Write your ASHA number inside the cover of a notebook and bring the notebook with you as your note-taking paper for all CE activities. 4. Louisiana Speech-Language-Hearing Association 2009 Convention June 10-12, 2010 Baton Rouge, LA LSHA Continuing Education Listing Policy Adopted: September 27, 2003 (amended: September 13, 2008) A calendar of events relevant to the contemporary practice of audiology and speech-language pathology may be posted on the LSHA website and in the newsletter free of charge for nonprofit organizations as well as universities. Practitioners who are conducting not-for-profit activities may also post meetings and workshops on the calendar of events. The calendar may include the title of the activity, date, location, and contact person. Private, for profit activities, whether individually or corporate sponsored, will be required to pay CE listing rates (LSHA Advertisement Request Form). To post an event please contact the LSHA Office. Organizations or groups seeking co-sponsorship of events for ASHA CEU approval should direct questions to: Kimberly Stafford, SLP Promotional materials for the event must be pre-approved by the CE Administrator and must include the required ASHA CE promotional paragraph and CE logo1/CE logo2.
